BMDAC visited YiWu Hospital Affiliated to Hangzhou Medical College
Oct 22-25, 2023
This year is the 12th year that the Bethune Medical Development Association of Canada has carried out medical exchange projects with hospitals of China. Fortunately, this is my second time to participate.
Dr. Ying Tung Sia, who accompanied me, is the director of the Cardiovascular Research Department, director of the Department of Cardiology at the Trois-Rivieres Central Hospital affiliated with the University of Montreal in Canada, and a cardiologist at the Quebec Heart and Lung Center. He is specialized in cardiovascular intensive care, ECMO, cardiac ultrasound, interventional AV, etc. Ultrasound experts.
The first stop of our trip was YiWu Hospital Affiliated to Hangzhou Medical College. During the three-day trip, both the host and the guest gained a lot.
The three-day itinerary was packed. We participated in the discussion of two critical and difficult cases in the ward; In the hospital function hall, the professor gave a lecture on transcatheter aortic valve replacement and the application of anticoagulant drugs in atrial fibrillation; in the cardiology clinic, the professor participated in the formulation of long-term treatment plans for three patients with recurrent symptoms after stents surgery; in the cardiac catheterization laboratory, the professor participated in the diagnosis and formulation of postoperative plans for four patients with coronary angiography; In the emergency department, the professor provided treatment to two cases of severe cardiopulmonary disease Treatment suggestions were provided to patients with the disease; and finally, the actual operation process of cardiac ultrasound was watched in the ultrasound room.
In the process of participating in the analysis of cardiology cases, colleagues from YiWu Hospital deeply realized the clinical guiding significance of cardiac ultrasound. The situation in China is that cardiac ultrasound is located in ultrasound departments other than cardiology and emergency departments, which is completely different from Canada. The disadvantages caused by dissociation are heart structure, valves, and the diagnosis of heart failure completely relies on a third departmentwhich is not focus on cardiac department. This greatly limits the accuracy of disease diagnosis. For example, in a patient whose heart failure was thought to be caused by coronary heart disease, our on-site ultrasound evaluation revealed that valve disease was actually the primary cause. Director Li from the Department of Cardiology said with emotion after we analyzed several cases that he had actually realized this for a long time. He shared that he often had this experience, watching patients whose heart failure symptoms improve day by day. But the Ejection FractionScores did not change or even dropped, which often caused problems for them in further treatment. We found the problem during a visit to the ultrasound room on the last day. The ejection fraction value provided by sonographers still relies on the traditional one-dimensional M-mode method, while the current common standard measurement is the three-dimensional volume biplane method. Director Li said with emotion that their goal is to build their cardiology department into a distinctive department of the hospital. The development of cardiac ultrasound is imperative. The professor Sia said that in the follow-up process, if necessary, we can do our best to provide help.
The cooperation of doctors across countries and languages is the reappearance of Bethune's spirit, selflessness and dedication.
